Form for Non-U.S. Citizens visiting General Atomics
(must be submitted by March 28, 2007

For those persons visiting General Atomics for the ITPA meeting and who are not U.S. citizens, the following form has to be filled out and submitted before April 19. 2007.

General Atomics is obligated to comply with the U. S. Department of Energy policy 142.1 for persons visiting the site who are not citizens of the United States of America. All such persons must have approval prior to visiting the Energy Group at General Atomics. If approval is not received, the non-U.S. citizen could be subject to not being allowed access to the GA site. The form below is only for persons who are not U. S. citizens. If you are a U. S. citizen you do not need to complete this form.

Note that this form can be submitted after March 28, 2008, but then there will be no guarantee that the approval process will be completed prior to the meeting dates. So submit this form by March 28, 2008 even if you do not completely know that you will be attending the meeting.
